In this fully remote role, you will provide comprehensive telehealth care to patients living with Alzheimer’s disease and other dementias, partnering closely with cognitive neurologists, geriatricians, behavioral health clinicians, and care coordinators to deliver coordinated, high-quality virtual care. You will diagnose, treat, and manage dementia and cognitive disorders while supporting patients and caregivers through education, medication management, and care planning.
Play a pivotal role in Vynca's Clinical Care Delivery Team as a Registered Nurse. You'll deliver top-notch, evidence-based care to seriously ill patients through telemedicine. This role involves managing patient care, communicating via phone and video, developing care plans, managing medications, coordinating with the interdisciplinary team, and educating patients and caregivers.
As a Case Management Nurse I, you will serve as a key advisor who actively engages with members through a variety of channels to foster a caring and trusting level of connection in order to support their chronic and/or complex health care needs and concerns. You will create and manage unique, individualized care plans that ensure the best possible health care goals and complex health needs are reasonably met. You will partner with members, members’ families, health care providers and community resources to coordinate and facilitate care and services.
The Triage Nurse is a remote Registered Nurse who provides telephone and electronic triage support to firsthand individuals and staff, while also supporting outpatient care coordination. This is primarily a day-shift role, with occasional potential for nights or weekends. Manage inbound clinical issues from firsthand staff and patients via phone; triage appropriately and escalate emergencies immediately. Work closely with peer mental health workers, social workers, and APPs to address acute issues comprehensively.
