Privia is seeking a forward-thinking, organized, and energetic Nurse Care Manager to drive change within the medical group and affiliated partners. This role involves extensive care coordination for Privia’s highest risk patients, working with population health teams, networked facility partners, primary care providers, and practice operations. The Nurse Care Manager will engage patients telephonically, in office, in facility, or where needed, acting as a liaison between the patient and Value-Based Care Programs.

Empower individuals facing health challenges by providing compassionate, expert-guided care advocacy services. As a Patient Advocate, you will act as a liaison between patients with chronic conditions and the healthcare system, ensuring comprehensive and coordinated care. This role focuses on empowering patients to make informed decisions about their health.

This fully remote position as a Chronic Care Manager plays a key role in supporting patients with chronic conditions by providing proactive, ongoing care coordination and education.
